Severity of Alcohol Dependence Questionnaire (SADQ)

The SADQ is a short, easy-to-complete, self-administered, 20-item questionnaire designed to measure severity of dependence on alcohol. The SADQ is a fairly specialised test, and it is aimed at people who have a drink problem in order to measure the level of addiction. It does not look at any other areas of alcohol related harm.

Please do not attempt the SADQ unless you have already completed the DRINKULATOR test.

This section contains questions which will give you a detailed picture of whether you are dependent on alcohol - what used to be known as an alcoholic.
